Job summary

Crossover Health is seeking a Physician to lead the care team and provide outstanding, comprehensive primary health for a panel of patients. The Physician offers medical oversight, expertise, and leadership to ensure the delivery of high-quality, affordable care within our innovative Primary Health model.

Qualifications

• Residency in Family Medicine or Internal Medicine (ACGME approved).
• Active state medical license (valid on start).
• Current BLS (Basic Life Support) certification.
• Current DEA Registration.
• Three professional references.

Responsibilities

• Provides patient-centered, comprehensive primary care services consistent with the patient-centered medical home model.
• Provides continuous availability for medical and technical questions to patients, care staff, and ancillary providers.
• Provides after-hours coverage in rotation with other market clinicians.
• Participates in practice quality improvement programs and evaluates practice performance metrics.
• Adheres to privacy, compliance, and quality programs and our Code of Conduct.
• Participates in implementation of new health technologies, products, services, and programs.
• Performs other duties as assigned.
