Responsibilities

• Evaluate, diagnose, and manage patients with acute illnesses, chronic diseases, and preventive healthcare needs through comprehensive history, physical exams, and evidence-based medical decision-making.
• Develop and oversee individualized treatment plans including medications, preventive services, immunizations, health screenings, and chronic disease management.
• Coordinate care with specialists, APPs, nurses, pharmacists, social workers, and care managers to optimize patient outcomes within value-based care initiatives.
• Educate patients and caregivers on diagnoses, medications, preventive health, nutrition, exercise, and self-management of chronic diseases.
• Document encounters and outcomes in the EHR, ensuring compliance with regulatory and payer guidelines.
