Job summary

Qualifications

• NP degree is required.
• Post-master certificate or higher required.
• Accredited NP program by ACEN/CCNE required.
• Active NP license in the US required.
• NP board certification required.
• Foreign education evaluated if applicable.

Responsibilities

• Independently manages a panel of patients in primary care.
• Develops and implements care plans for population health.
• Demonstrates leadership within the primary care team and interdisciplinary colleagues.
• Orders and interprets labs and diagnostic tests.
• Prescribes pharmacologic and non-pharmacologic interventions.
• Initiates referrals to specialty services and provides patient education.
