Job summary

AbsoluteCare is seeking a Lead Physician, Primary Care in Cincinnati to implement team-based, population health approaches for medically complex and socially vulnerable members. The role emphasizes collaboration with an interdisciplinary team, mentoring colleagues, and leading clinical care within the center and market leadership.

Responsibilities include managing a care panel, guiding care teams, and partnering with community and payer partners to achieve quality and utilization goals.

Qualifications

• Active, unrestricted medical license in the state of employment.
• Clinical and administrative experience leading interdisciplinary teams.
• Experience with training and mentoring.
• Experience working for a clinic, community-based organization & inpatient unit.
• Strong problem solving, project management and organization skills.
• Independent self-starter and critical thinker interested in population health and care model development.

Responsibilities

• Provide excellent primary care to a panel of members with chronic conditions and post-acute needs.
• Lead and collaborate within an interdisciplinary team to implement and adjust patient care plans.
• Coach and mentor physicians and APPs on value-based and evidence-based care.
• Address questions and provide clinical guidance to nurses and other providers.
• Serve as clinical leader in the center and participate in market leadership meetings.
• Ambassador to community organizations, hospital systems, and payer partners.
