Enhance Rehabilitation in Chicago, IL is seeking qualified Physical Therapists to join our SNF team. You will evaluate and treat patients following an individualized plan of care.

Responsibilities include supervising Physical Therapist Assistants, re-evaluating progress, documenting evaluations and progress notes, and collaborating with facility staff to ensure high quality patient care.

• Collaborate with facility staff to ensure excellent care for you patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
