Responsibilities

• Provide evaluation and treatment following an individualized plan of care.
• Deliver excellent care and customer service to patients and families.
• Collaborate with facility staff to ensure high-quality patient care.
• Evaluate new patients, develop plans of care, and set functional goals.
• Treat patients, address concerns, and educate on injury, progress and outcomes.
• Supervise Physical Therapist Assistants.
• Re-evaluate progress, revise plans of care, and adjust goals.
• Complete timely documentation including evaluations, re-evaluations, discharges, and daily progress notes.
