Responsibilities

• Plan, organize, develop, and provide Physical Therapy Services.
• Participate in discharge planning and resident care planning.
• Maintain treatment records and progress notes.
• Conduct quarterly therapy screens to determine service need.
• Collaborate with medical staff and departments; ensure resident safety.
