Job summary

Bremertontrailspa seeks a licensed Physical Therapist to provide and direct all aspects of patient care in a long-term care setting.

You will evaluate, plan treatment, educate families, document progress, and collaborate with the rehab team to ensure safety and quality care for residents.

Experience in Medicare payor understanding and documentation accuracy is preferred; strong communication and empathy are essential to support residents and their families.

Qualifications

• Graduate of an approved Physical Therapy program.
• Active Physical Therapist license in good standing.
• CPR certification.
• Long-term care experience preferred.
• Proficient in English.

Responsibilities

• Evaluate patients per MD orders and payer guidelines.
• Develop and implement individual treatment plans.
• Educate staff, patients, and families on intervention benefits.
• Complete evaluations, progress notes, and monthly summaries as required.
• Direct and supervise assistants and rehab aides within state/federal guidelines.
• Maintain patient safety and adhere to organizational protocols.
