Job summary
The Nesconset Center for Nursing & Rehabilitation is seeking an experienced Physical Therapist to assess patients, develop and administer rehabilitation plans, and educate patients and families. This role includes supervising PT assistants and collaborating with the caregiving team to deliver optimal, patient-centered care.
Candidates should have a PT degree from an accredited program, a valid state license, and experience in long-term or geriatric care.
Qualifications
- Graduate from an accredited Physical Therapy program.
- Valid physical therapy license and registration for state of practice.
- Experience supervising Physical Therapy Assistants.
- Experience in long-term care and/or geriatric care preferred.
- Strong documentation writing skills.
- Experience with case mix index and PPS scheduling is a plus.
Responsibilities
- Assess referred patients and provide direct PT care.
- Establish and administer treatment programs with goals.
- Document patient status and educate patients and families.
- Collaborate with all disciplines for patient-centered care.
- Maintain punctuality and professionalism in duties.
