Glengariff Healthcare Center in New York is seeking a dedicated Physical Therapist to assess referred patients requiring rehabilitation and to design individualized treatment plans. You will set goals with physician input and document progress while educating patients and families in appropriate PT methods.

The role includes supervising Physical Therapy Assistants, performing evaluations, weekly progress notes, recertifications and discharge documentation, and collaborating with all disciplines

Formación

• Must have graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of NY.
• Experience supervising Physical Therapy Assistants and documenting patient progress.

Responsabilidades

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals per patient capacity.
• Document patient status, educate patients/families, and ensure timely discharge documentation.
• Collaborate with all disciplines to deliver patient-centered rehab in LTC/long-term care.
