Glengariff Healthcare Center in New York is seeking a Physical Therapist (PT) to assess referred patients, develop and administer rehabilitation plans, and educate patients and families on therapy methods. The role requires state licensure and experience in geriatric care.

Work is on-site within a premier sub-acute and long-term care setting. The PT will document patient progress, set achievable goals, and collaborate with the rehabilitation team to provide patient-centered care.

Qualifications

• Graduated from an accredited Physical Therapy college or university.
• Must hold a valid NY professional license and registration.
• Overseeing and supervising Physical Therapy Assistants.
• Experience with long-term care/geriatric care is preferred.
• Well versed with documentation writing.

Responsabilités

• Assess referred patients to determine rehabilitation needs.
• Establish and administer treatment plans with goals based on patient capacity and tolerance.
• Document patient condition and progress weekly; discharge documentation.
• Educate patients and families in appropriate physical therapy methods.
