Job summary

Qualifications

  • PharmD degree required from an accredited college of pharmacy.
  • 1–2 years pharmacy experience in retail, mail order or specialty pharmacy.
  • Maryland Pharmacist license with Immunization/Vaccination Certification; CPR/BLS/NRP required.

Responsibilities

  • Evaluate prescriptions for dose appropriateness, efficacy, allergies, interactions and contraindications.
  • Process insurance pre-authorizations and manage DUR rejects; ensure accurate dispensing and labeling.
  • Conduct medication reconciliation and provide patient education to improve adherence and outcomes.
  • Document patient care activities per accreditation and payer requirements.
