Job summary

Hello Neighbor in Pittsburgh seeks a Lead Case Manager – Smart Start to oversee perinatal case management, supervise staff, and ensure program quality across Allegheny County, Washington County, and Charleroi. You will coordinate services for pregnant clients, provide trauma‑informed care, and support cross‑site collaboration.

The role requires travel, evening/weekend flexibility, and a willingness to work in a diverse refugee and immigrant community.

Qualifications

• Preferred Qualifications include: perinatal or maternal–child health case management with vulnerable populations.
• Experience in a lead or supervisory role coordinating staff and workflows.
• Trauma‑informed, culturally responsive care and perinatal best practices.
• Experience overseeing programs across multiple sites or geographic areas.

Responsibilities

• Maintain an active caseload of pregnant clients in the Smart Start program in Allegheny County, including referrals, intakes, assessments, and service plan implementation.
• Provide trauma‑informed perinatal case management from pregnancy through postpartum.
• Support prenatal, birthing, and postpartum healthcare navigation and follow‑ups.
• Provide perinatal education and birth planning in collaboration with clients and providers.
• Supervise the Wellness Liaison – Haitian Community and Smart Start Case Aide; support onboarding and staff development.
