Hamilton Health Center in Harrisburg, PA, seeks a Grant-Funded Healthy Start Case Manager to deliver comprehensive case management to prenatal, postpartum, and parenting families enrolled in Healthy Start.
You will connect families to medical care and community resources, support healthy behaviors around pregnancy and early child care, and maintain documentation per funding requirements. A Bachelor’s degree and CPR/First Aid certification are preferred; travel within Dauphin County is required.
The Grant-Funded Healthy Start Case Manager is responsible for delivering comprehensive case management services to eligible prenatal, postpartum, and parenting families enrolled in Hamilton Health Center’s Healthy Start program. Healthy Start serves pregnant women and families with children up to 18 months of age who reside in Dauphin County. The Case Manager works to improve access to medical care and community resources, promote healthy behaviors before, during, and after pregnancy, and support safe and healthy infant and toddler care practices through a patient-centered, community-based approach.
