Community Bridges, Inc. leads a multidisciplinary care team to provide direct patient care in behavioral health settings across Arizona. The Lead PCT coordinates with RNs, PCTs, and BHMPs to maintain a safe environment and delivers high-quality care to patients presenting for services.

The role requires responding to crises, supervising staff during interventions, and ensuring compliance with program policies. A healthcare background and a commitment to compassionate patient care are essential.

Qualifikationen

• Highschool diploma or GED required. Associate degree or higher preferred.
• One (1) or more years of health care related experience where his or her prior experience would provide adequate exposure to both behavioral and medical crisis situations required.
• Emergency Medical Technician certification (valid within the state of Arizona) is preferred but not required for this position.
• Certified Nurse Assistant certification (current and valid within the State of Arizona) is preferred but not required of this position.
• Valid identification required.

Aufgaben

• Coordinate with RN, PCT, and BHMP to ensure a safe environment and quality care for all presenting for services.
• Provide direct patient care when needed and support to the PCTs on their shift.
• Identify environmental and patient-specific safety concerns and direct staff during non-violent physical interventions and seclusion/restraint episodes.
• Ensure full knowledge of program policy and procedures and maintain unit safety and operational effectiveness.
