Job summary

Central Valley PACE in Merced is seeking a Home Care Personal Care Assistant II. The role leads patient care activities, mentors PCA staff, and coordinates with nursing teams while serving clients across Merced County.

This full-time home care position requires travel, a valid CA license, and relevant certifications. The role supports elderly participants in their homes with daily living activities and health services.

Qualifications

• Valid California Driver’s License and vehicle insurance.
• Reliable transportation and ability to travel across Merced County.
• Experience with geriatric care and home care settings.

Responsibilities

• Act as mentor to PCAI and train staff as needed.
• Communicate patient care needs to LVN, RN supervisor, and Manager.
• Deliver basic personal care and activities of daily living.
• Perform COVID and Influenza testing when assigned.
• Documentation of treatments and observations in EMR.
