Job summary

Central Valley PACE in Modesto, CA is seeking a Part-Time PACE Personal Care Assistant II for the Day Center. The role leads PCA duties and delivers patient care activities while supporting nursing staff.

Schedule is Monday–Friday, 10:00am–2:00pm with compensation of $24.31–$25.53 per hour. Requires valid CA driver’s license, geriatric care ability, and CA licensure (CNA/HHA/EMT or equivalent); BLS CPR required. Training provided as needed.

Qualifications

• Requires valid California driver’s license and acceptable driving record.
• Must be able to treat people of all ages including geriatrics.
• Meet CMS-approved competency standards before working independently.

Responsibilities

• Communicates patient care needs to LVN and supervising RN.
• Provides direct personal care and assistance with ADLs as directed.
• Assists in training new staff and PCs as needed by management.
• Maintains patient areas clean and organized; supports meal/snack service.
• Documents treatments, observations, and activities in the EHR.
• Participates in IDT meetings and care planning.
