This RN provides patient care, care coordination, health promotion, education and coaching on wellness, disease prevention, and chronic care management in a specialty outpatient setting. Key responsibilities include policy development, performance improvement, peer feedback, and mentoring colleagues to deliver safe, high-quality care.

Qualifications

• Full and unrestricted license from any US State or territory.

• Collaborates in development, implementation, evaluation, and revision of policies, procedures, and/or guidelines.
• Provides peers with constructive feedback for improvement and supports knowledge sharing to provide safe, quality specialty nursing care.
• Fosters a safe and supportive environment for professional development and evaluates outcomes of care decisions at the program or service level.
