Job summary

Avir at Fort Worth is seeking a Nurse Practitioner (Skilled Nursing) to serve as a clinical leader in our SNF. You will provide internal medicine care, collaborate with physicians and nursing staff, and manage the health needs of short- and long-term residents.

Responsibilities

• Lead internal medicine care as SNF’s primary NP.
• Collaborate with interdisciplinary team and physicians.
• Supervise daily patient care for short- and long-term residents.
• Conduct new admission evaluations and discharge planning.
• Perform monthly stability visits for long‑term residents.
• Provide comprehensive health assessments and differential diagnoses.
• Prescribe treatments under facility protocols and manage medications.
