Job summary

Avir Health Group in Texas seeks a Nurse Practitioner (Skilled Nursing) to lead internal medicine services in a SNF setting. You will collaborate with an interdisciplinary team and guide daily resident care with nursing staff and medical directors.

You will evaluate admissions, conduct discharge and stability visits, and prescribe treatments while ensuring comprehensive patient assessments and care plans in a fast-paced environment.

Qualifications

• APN Texas license and active DEA number required.
• Master’s degree in nursing or related field mandatory.
• Previous nursing/medical field experience preferred.
• Excellent written and verbal communication and leadership capabilities.

Responsibilities

• Lead internal medicine services as SNF's primary provider.
• Collaborate with interdisciplinary team and physicians daily.
• Oversee day-to-day care for all residents with nursing staff guidance.
• Evaluate new admissions and coordinate discharge visits.
• Conduct monthly stability visits for long-term residents.
• Perform comprehensive physical exams and manage acute/chronic illness.
• Prescribe treatments, medications, and develop patient management policies.
• Facilitate referrals and communicate with attending physicians.

Major Responsibilities

• As the SNF-ist, you will act as a Skilled Nursing Facility’s leading provider of internal medicine services
• You will collaborate with the facility’s interdisciplinary team and physicians, serving as a day-to-day clinical leader while helping oversee the care of all of the facility’s residents
• You will work with the entire facility’s patient population - short and long-term residents - overseeing their day-to-day care with the assistance of the facility’s nursing staff and guidance from the facility’s medical directors and attending physicians
• Evaluation of new-admission visits
• Coordination of discharge visits
• Monthly stability visits for long term residents
• Evaluations for patients exhibiting changes in condition
• As a SNF-ist, you will provide types of care, including but not limited to:
• Advanced Care Planning
• Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease
• Prescribe all necessary medications and treatments
• Perform comprehensive physical examinations of patients
• Develop and implement patient management policies and procedures
• Facilitate referrals to other healthcare professionals and medical facilities
• Communicate with attending physicians and/or specialists regarding patient care

Qualifications

• Advanced Practice Licensure (APN) with a Texas license
• Must have an active license and DEA number.
• Previous experience in nursing or other medical fields.

• Excellent written and verbal communication skills.
• Strong leadership experience with emphasis on teamwork in a healthcare setting.
• Master’s degree required
