Job summary

Avir Health Group is seeking a Nurse Practitioner to lead clinical care in a Skilled Nursing Facility in Texas. You will work with the facility care team to manage admissions, discharges, and ongoing assessments.

This role includes evaluating changes in condition and participating in advanced care planning and prescribing treatments as needed. The ideal candidate holds Texas APN licensure with a DEA license, has nursing or clinical experience, and demonstrates strong communication and teamwork

Qualifications

• Active Texas Advanced Practice Licensure (APN) and DEA License.
• Previous nursing or clinical experience.
• Strong communication and teamwork skills.
• Master's degree in Nursing.

Responsibilities

• Oversee admissions and discharges for SNF residents.
• Perform monthly stability checks and evaluations for changes in condition.
• Lead advanced care planning and comprehensive assessments.
• Prescribe treatments as appropriate within SNF responsibilities.
