Guardian Angels Senior Services in Elk River, MN seeks a Lead MDS & Quality Coordinator (RN) to oversee MDS assessments, QA/QAA processes, and quality improvement initiatives on the Transitional Care Unit.
The role provides mentorship to other MDS Coordinators, partners with therapy teams, and ensures regulatory compliance, staff education, and data-driven care for residents.

Salary Range: $80,000 - $104,000 (Based on Experience)

Provide accurate and complete assessment and documentation of residents consistent with parameters and definitions established for current MDS format. Assists with resident admissions on the Transitional Care Unit. Responsible for Quality Assurance Assessments and other related functions. Services are provided in accordance with the mission of Guardian Angels.
