Job summary

Garden City Health Care Center located in Modesto, CA is seeking a Licensed Vocational Nurse (LVN) to join our skilled nursing team. This full-time role includes leadership responsibilities supervising CNAs and coordinating care across shifts.

Candidates must hold a California LVN license and CPR certification, with strong documentation and communication skills. Prior long-term care experience is preferred, and PCC knowledge is a plus.

Qualifications

• Current LVN license in good standing with the California Board of Nursing.
• CPR certification required.
• Experience in a skilled nursing or long-term care setting strongly preferred.
• Strong documentation, leadership, and communication skills.
• Knowledge of PCC (PointClickCare) preferred.

Responsibilities

• Supervise and direct Certified Nursing Assistants during assigned shifts
• Oversee daily operations and ensure adherence to care plans and facility policies
• Participate in staff evaluations, orientation, training, and quality assurance initiatives
• Communicate effectively with residents, families, physicians, and interdisciplinary teams
• Respond to incidents, complete documentation, and elevate concerns to Nurse Supervisor
• Administer medications and treatments per physician orders
• Monitor and chart resident health status and care interventions
• Support admissions, discharges, and transfers with proper documentation
• Participate in care planning and discharge planning processes
• Maintain accurate records in compliance with state, federal, and facility requirements
• Enforce hand hygiene, infection control, and safety procedures
• Participate in emergency preparedness and fire/disaster drills
• Maintain a clean, safe, and sanitary work environment
• Ensure proper use of PPE and disposal of medical waste
