Job summary

Sunharbor Manor in New York State seeks an experienced LPN to provide direct nursing care and supervise nursing assistants under the physician’s guidance. You will ensure policy adherence, plan shift services, and participate in surveys to uphold quality care.

Qualifications

• Must have a current, unencumbered, active LPN license in New York State.
• Must possess the ability to make independent decisions when circumstances warrant such action.
• Knowledgeable of nursing and medical practices and long-term care Regulations.
• Leadership and supervisory ability to work harmoniously with and supervise other personnel.

Responsibilities

• Ensure nursing personnel comply with written policies and procedures.
• Plan shift services, programs, and activities with assigned staff.
• Participate in facility surveys by government agencies as requested.
• Admissions, transfers, and discharges of residents as required.
• Complete accident/incident reports as necessary.
• Maintain the daily report.
• Perform administrative duties such as forms, reports, and charting.
• Receive physician telephone orders and record as policy requires.
• Administer medications per physician orders and facility policy.
• Count controlled drugs with oncoming shift and maintain records.
• Orient new employees to shift policies and procedures.
• Attend and participate in in-service education programs.
• Monitor safety and infection control procedures among staff.
• Maintain confidentiality of resident care information.
• Respect resident rights and dignity.
• Assess basic physical, emotional, and social needs of residents.
• Collect data for admissions assessment.
• Develop and update resident care plans.
• Assist residents with activities of daily living.
• Safely perform therapeutic nursing techniques per assessment.
• Document resident care and response.
• Educate residents and families on health habits.
• Manage care of residents on the unit.
• Recognize and respond to changes in residents' conditions.
• Communicate with doctor about changes and accept telephone orders.
• Maintain compliance with NYS DOH LTC survey standards.
• Participate in interdisciplinary team meetings.
• Complete incident reports and assist investigations.
• Assign personnel and direct unit function.
• Document monthly summaries and LTC-specific reports.
• Report unit and resident conditions to nurse manager.
