Job summary

Qualifications

Responsibilities

• Coordinate day-to-day resident care per service plan.
• Oversee staffing and supervise RMAs and direct care staff.
• Provide information to care planning staff about residents’ needs.
• Coordinate services with hospice and home health as ordered.
• Coordinate medication management with pharmacies and document care.
• Report changes in residents’ condition to the care team and families.
