Job summary

Chestnut Woods Rehabilitation & Healthcare Center in Massachusetts seeks an experienced LPN to join a compassionate care team. A minimum of 1 year of nursing experience is required; this PRN position offers daily pay and a shift differential up to $3.80 for evenings and nights.

Responsibilities include medication administration, following care plans, documenting accurately, and communicating changes to the health team, while supporting residents’ safety and dignity in a warm, home-like facility.

Qualifications

• Active LPN license in MA.
• BLS/CPR certification required.
• Ability to care for multiple patients in a fast-paced environment.
• Strong clinical judgment and communication skills.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of Massachusetts.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.
