Job summary

Signature HealthCARE is seeking a caring LPN/LVN to provide direct nursing care to residents at Colonial Rehab & Wellness Center in Bardstown. You will administer medications, perform wound care, and support ADLs within a skilled nursing environment.

The role requires an active LPN/LVN license in Kentucky, CPR/BLS certification, and at least one year of clinical nursing experience (long-term care preferred). Supervisory duties and strong documentation are a plus in this team-focused culture.

Qualifications

• Current, active LPN/LVN license in the state of practice (Kentucky).
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
• Monitor and observe residents’ physical and emotional status; notify physician and responsible party of changes.
• Develop, implement, and review individualized resident care plans with interdisciplinary team.
• Complete accurate and timely clinical documentation per policy.
• Coordinate resident admissions, transfers, and discharges.
• Participate in quality assurance activities, staff education, and regulatory surveys.
• Perform other duties as assigned by Director of Nursing.
