Signature HealthCARE of Memphis seeks an experienced LPN/LVN to provide direct nursing care to residents, including medication administration and wound care, while supervising nursing staff and ensuring safety and quality care. You will participate in care planning and ensure accurate documentation in line with facility policies.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Experience in a supervisory or charge nurse role.
• Strong clinical observation, medication administration, and documentation skills.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides during the shift.
• Monitor residents’ status and notify physician of changes.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete timely clinical documentation per policy and procedure.
• Coordinate admissions, transfers, and discharges per procedures.
• Participate in quality assurance, staff education, and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing.
