Prairie Path Senior Living in Lombard, IL is seeking a Licensed Practical Nurse to collaborate with residents in developing care plans and supervising staff to maintain maximum function.

Qualifications

• 1 year of nursing experience and a current Illinois LPN license.
• Supervisory experience preferred.
• Knowledge of Illinois State Assisted Living guidelines and regulations.
• Strong communication to lead, orient staff and interact with residents and families.
• Strong time management and problem solving skills.
• Must be able to read, speak, and understand English.

Responsibilities

• LPN provides direct care to residents and assessment in emergencies.
• Schedules lab tests, orders lab work, checks results, and keeps families and charts updated.
• Evaluates resident function and status at admission and during changes.
• Ensures staff carry out non-skilled care tasks and maintains high care quality.
• Delivers prompt feedback to staff and develops service plans.
• Maintains documentation per unit policies and state regulations.
