Job summary

Charter Oak Communities in Stamford, CT is seeking a Licensed Practical Nurse (LPN) to lead daily clinical operations under the RN/Nurse Manager. You will ensure compliant documentation, medication administration, and care plan execution in a residential care setting, while supporting a resident-centered environment.

The ideal candidate has a CT LPN license, CPR certification, and at least four years of LPN experience (two in supervision), with geriatric or behavioral health experience

Qualifications

• High school diploma required; Associate degree preferred.
• Minimum four years LPN experience, two in managerial roles preferred.
• Experience in geriatric, behavioral health, and/or residential care settings preferred.
• Active Connecticut LPN license required and CPR certification.
• Proficiency with electronic medical records and MS Office.

Responsibilities

• Lead and manage day-to-day clinical care delivery for residents under RN/Nurse Manager oversight.
• Coordinate care plans, assessments, and documentation in real time.
• Oversee medication administration and MAR/eMAR accuracy.
• Provide staff guidance and training to CNAs and care staff.
• Ensure regulatory compliance with Connecticut DPH and organizational policies.
• Communicate with residents, families, physicians, and providers to ensure coordinated, high-quality care.
