Ohio Living Swan Creek is seeking a Charge Nurse - LPN to supervise CNAs and oversee daily nursing care in our long-term care setting. You will administer medications, coordinate resident plans, and ensure regulatory compliance while delivering compassionate, patient-centered care.

We seek an Ohio-licensed LPN with at least one year of clinical experience and supervisory skills. This full-time role may include PRN opportunities, with a collaborative team and opportunities for professional growth.

Qualifications

• Current unencumbered Ohio LPN license.
• One year of clinical experience in a hospital, long-term care facility, or related health care setting preferred.
• Supervisory experience in a long-term care, hospital, or related healthcare setting preferred.
• Proficiency with electronic medical records (EMR) preferred.
• Must be able to read, write, speak, and understand the English language.

Responsibilities

• Directs day-to-day nursing activities in accordance with rules, regulations, and guidelines governing the facility.
• Administers medications and treatments per physician orders and standards of practice.
• Represents the nursing department in Interdisciplinary Care Plan meetings and answers questions about the plan of care.
• Supervises CNAs, assigns work, and identifies areas for improvement in care and services.
• Ensures quality, compliance, and risk management through QA processes and policy adherence.
