Job summary

Trilogy Health Services, LLC is seeking an Assisted Living Coordinator in Monticello, IN. The role oversees resident assessments, medication management, ADL support, and care planning in collaboration with nursing leadership.

You will coordinate with the DON/ADON to ensure smooth clinical operations on campus. Responsibilities include pre-admission screens, ADL assistance, and leading service plan development with residents and families; participation in on-call rotations is required.

Qualifications

• Active, unencumbered LPN license within Indiana.
• CPR certification required.
• Previous assisted living and leadership experience preferred.

Responsibilities

• Collaborates with the Director of Nursing (DON) and Assistant Director of Nursing (ADON) regarding day-to-day campus clinical services.
• Ensures pre-admission screen is completed prior to move-in.
• Directs lifestyle and care needs assessments to determine residents' medical and personal care requirements.
• Assists residents with activities of daily living and medication administration.
• Leads service plan development with resident/family and communicates plan to caregivers.
• Facilitates Assisted Living Resident Meetings and follows up with feedback and concerns.
• Works as lead nurse for assisted living residents and participates in on-call rotation.
