Qualifications

• Active, unencumbered LPN license in Indiana.
• CPR certification required.
• Previous assisted living and leadership experience preferred.

Responsibilities

• Collaborates with the Director of Nursing (DON) and Assistant Director of Nursing (ADON) regarding day-to-day clinical services for the campus.
• Ensures that each resident’s pre-admission screen is completed prior to move-in.
• Directs the Lifestyle and Care Needs Assessments to determine residents' medical and personal care needs.
• Assists residents with activities of daily living (ADL) and medication administration.
• Leads development of the service plan with resident/family and communicates the plan to caregivers.
• Facilitates the Assisted Living Resident Meeting and follows up on feedback and concerns.
• Acts as lead nurse for assisted living residents and participates in on-call rotation.
