Sierra Vista Hospital in Sacramento seeks Licensed Therapists for the Outpatient Department. The Lead Clinician acts as the primary contact in the absence of the Program Manager and mentors other clinicians, delivering direct treatment and ensuring care aligns with individualized plans.

The role includes case management, discharge planning, referrals, and coordination with physicians, nurses, and aftercare services. A current LCSW/LMFT license is required; DBT training is preferred.

Qualificações

• Master's degree in social work or related field.
• Minimum of two years direct clinical experience in a psychiatric or mental health setting.
• Current LCSW or LMFT license in good standing.

Responsabilidades

• Lead Clinician provides direct treatment activities (individual, family, group or couples) and ensures services align with the patient’s treatment plan.
• Performs case management tasks including discharge planning, referrals, and coordinating aftercare services.
• Collaborates with attending physician, nurse and other staff; conducts utilization reviews and arranges appointments.
