Job summary

Matthew 25 AIDS Services in Kentucky seeks a dedicated case manager to provide medical case management under KHCCP, conduct social service assessments, and connect HIV+ clients with needed federal, state and local programs.

The role supports Housing First, ensures Ryan White eligibility, and coordinates care with housing and health resources. Travel within the state may be required; a driving record must be clean.

Qualifications

• Bachelor’s in Social Work or related field, or LPN certification.
• Basic knowledge of medical case management principles.
• Experience in nonprofit or service-driven environment preferred.
• Full commitment to Matthew 25's mission and values.
• Valid driver's license with clean record and travel willingness.
• Ability to pass drug screen and background check.

Responsibilities

• Provide direct case management and housing support to clients under KHCCP, HOPWA, TBRA and HUD guidelines.
• Serve as day-to-day resource for team support, scheduling and service delivery questions.
• Perform regular chart audits and documentation reviews for accuracy and compliance.
• Support staff training, onboarding, and skill development to maintain standards.
• Promote Housing First and connect clients to medical, housing, and social resources.
• Assist clients in maintaining Ryan White eligibility and health insurance access.
• Collaborate with Program Manager on workflow, performance monitoring and improvements.

Overview:

Provides medical case management services under the Kentucky HIV/AIDS Care Coordination Program (KHCCP), including social service assessments and connecting HIV+ individuals with needed services offered by Federal, State, and Local community programs.

Key Responsibilities

• Provide direct case management and housing support to a reduced caseload of clients in accordance with KHCCP, HOPWA, TBRA, and HUD guidelines.
• Serve as the go-to resource for day-to-day team support, assisting with scheduling, service delivery questions, and complex client situations.
• Conduct regular chart audits and documentation reviews to ensure compliance, accuracy, and consistency across the team’s caseloads.
• Support staff training, onboarding, and skill development to maintain program standards and service quality.
• Promote a Housing First approach and ensure clients are connected to appropriate medical, housing, and social service resources.
• Assist clients in maintaining Ryan White eligibility, accessing health insurance, and adhering to care plans and treatment.
• Collaborate with the Program Manager on workflow coordination, performance monitoring, and continuous improvement efforts.

QUALIFICATIONS

• Must have a Bachelor’s of Social Work or a B.S. in a related field, or have a Licensed Practical Nurse certification
• Basic knowledge of medical case management principles.
• Experience working in a nonprofit organization or a strong desire to work in service driven environment.

• Flexibility, as our services are available to a large geographic area in the state of Kentucky, which may require some travel
• Valid Driver’s License with at least three (3) years driving experience and less than two (2) moving violations in the past three (3) years.
