Job summary

Heart’n Soul Hospice in Seattle seeks an experienced Patient Care Manager – Registered Nurse to lead clinical departments, ensure the quality and safety of services, and coordinate care across sites in compliance with state and federal regulations.

The role requires a valid RN license, at least two years of hospice nursing, strong organizational and communication skills, and a proven ability to mentor staff within a multidisciplinary team.

Qualifications

• Strong leadership and coaching abilities are required.
• Excellent communication and organizational skills are essential.

Responsibilities

• Direct and coordinate clinical departments and sites, ensuring continuity, quality, and safety of services.
• Supervise nursing staff and guide multidisciplinary team to meet care plans.
• Provide education, coaching, and performance evaluations to staff.
• Ensure timely documentation and compliance with regulations.
