UnitedHealth Group's Assured Home Health unit seeks a Home Health Patient Care Manager in Washington to oversee clinical services and lead an interdisciplinary team in line with physician plans of care and state/federal rules.

The role emphasizes ensuring high-quality, individualized patient care, coordinating benefits, and maintaining seamless care delivery across the home health workflow. Strong nursing licensure and reliable transportation are required.

Qualifications

• Current unrestricted RN licensure in state of practice.
• Current driver’s license and access to dependable vehicle or public transport.
• WA-specific licensure restrictions must have none.
• Preferred: CPR certification or ability to obtain within 90 days.
• Home health care experience preferred.

Responsibilities

• Provides clinical services within the scope of nursing practice per plan of care, coordinating with the health care team.
• Receives referrals, assigns clinicians, evaluates orders, and starts care.
• Coordinates determination of home health benefits and medical necessity, and ongoing approvals.
• Ensures care is patient-centered and compliant with physician orders and eligibility criteria.
• Reviews assessments and plans daily, consulting clinicians as appropriate.
• Functions well in diverse patient situations regardless of background.
