Job summary

UnitedHealth Group is seeking a Home Health Patient Care Manager to oversee the clinical services for home health patients in the designated area, coordinating an interdisciplinary team and ensuring care meets physician plans and regulatory standards.

Responsibilities include managing referrals, scheduling start of care, determining benefits and approvals, and daily review of assessments to tailor patient care. RN licensure and reliable transportation are required; CPR preferred.

Qualifications

• Current unrestricted RN licensure in state of practice.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification preferred (within 90 days of hire).

Responsibilities

• Provides clinical services within scope of practice and in coordination with the health care team.
• Receives referrals, assigns clinicians, evaluates orders, and plots start of care visits.
• Coordinates determination of patient home health benefits and medical necessity.
• Ensures patient needs are continually assessed and care aligns with physician orders.
• Reviews assessments and plans of care daily and consults clinicians as needed.
