Qualifications

• Graduate of an approved school of professional nursing.
• Licensed in the state of practice or eligible under nurse licensure compacts.
• Minimum two years of nursing experience; one year in home health or hospice.

Responsibilities

• Coordinate the delivery of home health services to patients according to standards.
• Review and approve patient information during start of care, recertification, resumption of care, or evaluation visits.
• Facilitate relationships among physicians, referrals, patients, caregivers, and staff.
