Job summary

Merakey is seeking a Lead Direct Support Professional to supervise shifts, mentor staff, and ensure compliant care in a residential group home. You will support individuals with daily living activities, accompany them to appointments, and document progress in the electronic health record.

This on-site role requires a high school diploma (or GED), a valid driver’s license, and the ability to obtain an RBT certification within 60 days.

Qualifications

• High school diploma or GED is required.
• Certification – Registered Behavior Technician (RBT) or licensed psychiatric technician; or obtain RBT within 60 days.
• 6 months experience with IDD/MH population or equivalent.
• Must be at least 18 years old.
• Valid driver’s license, if required; ability to read/speak/write English.

Responsibilities

• Act as lead on assigned shift; serve as liaison for incidents and program concerns.
• Assist Administrator with staff scheduling as assigned.
• Support individuals in daily living activities and personal care in residential/community settings.
• Prepare meals meeting dietary needs; assist with medical appointments and paperwork.
• Dispense medications per policy; report errors immediately.
• Transport individuals per policy; perform housekeeping duties as needed.
• Document daily progress in the electronic health record and update logs.
• Report uncharacteristic situations to the manager for directions.

Lead Direct Support Professional

Position Summary

The Lead Direct Care Professional (DSP) is responsible for supporting individuals through teaching, coaching, modeling, and accompanying individuals with disabilities so that they are involved in family, neighborhood, civic, cultural and religious activities and events. In a variety of settings, the DSP will work with individuals with varying degrees of mental health diagnoses and abilities. The DSP upholds dignity and respect of the individuals served by promoting person‑centered care while documenting and assisting activities of daily living in a safe and sanitary environment.

PRN On Call, Varies – Days, nights, weekends as needed.

Compensation

Earn $26.00/hour. Professional development training and support to obtain a Registered Behavior Technician (RBT) Certification. Pay increases to $28.00/hour once RBT certification is obtained.

Administrative Responsibilities

• Act in lead role on assigned shift; point person for all activities, incident notification/reporting, and overall program responsible liaison.
• Assist Administrator in staff scheduling as assigned.

Direct Care Responsibilities

• Support individuals in tasks in a residential and/or community setting to include activities of daily living and personal care such as feeding, bathing, dressing, toileting.
• May prepare or assist with preparing meals that promote proper nutrition, good health and meet individual‑specific dietary needs.
• May accompany individuals to a variety of scheduled medical appointments, completing pre/post paperwork as needed.
• May dispense appropriate individual medication based on established policy and protocol; immediately report errors.
• May transport or arrange for transport of individuals, following vehicle policy and procedure and adheres to driving laws.
• Perform or assist, guide and support housekeeping functions.
• Document individual daily progress report in electronic health record; update all communication logs as directed.
• Immediately report to manager any uncharacteristic situation(s) for appropriate direction.

Support of an Individual’s Support Plan(s)

• Support and guide continued progress toward individual’s goals and outcomes.
• Follow any individual‑specific nursing care plan or behavior plan.
• Increase independence by facilitating, and promoting individual’s engagement in and use of, community resources, facilities and activities.
• As part of an interdisciplinary team, share information and observations in the development, review, and maintenance of the individual’s support plan.
• May support, guide and encourage individuals to access public transportation.

Compliance

• Comply with Merakey Policy and Procedures.
• Complete required documentation and correspondences in accordance with Federal, State, County, and Merakey policies and regulations.
• Follow emergency procedures as directed, i.e. Emergency Evacuation and Relocation Plan, inclement weather, etc.
• Perform all duties in accordance with HIPAA.
• Follow Merakey Code of Conduct.
• Complete, pass and maintain mandatory training.
