Job summary

The Opportunity Tree in Phoenix, AZ is seeking a Lead Direct Support Professional to support Members with developmental disabilities in developing basic living skills and promoting independence.

You will supervise health and safety, coordinate daily living activities and transport per the Person-Centered Plan, ensure medication procedures are followed, and document progress and outcomes for each Member. Collaboration with staff, families, and stakeholders is essential.

Qualifications

• Must be at least 18 years old.
• Driver license with clean MVR and insurer requirements.
• Fluent in English, written and spoken.
• Experience in IDD field preferred.
• High school diploma or GED.
• Basic MS Outlook, Word, Excel, and web browsing knowledge.
• Customer service mindset and teamwork abilities.
• Willing to complete required trainings (CPR/First Aid, etc.).
• AZ-DPS Fingerprint Clearance Card compliance.
• Reliable transportation; possible travel.
• Strong time management and problem-solving skills.

Responsibilities

• Supervise Members' health, safety, and daily living activities.
• Coordinate daily living skills and recreational events per Person-Centered Plan.
• Encourage development of positive interpersonal skills.
• Coordinate transportation as required by the plan.
• Ensure proper medication procedures are followed.
• Implement physicians' orders accurately.
• Develop, monitor, and document each Member's program.
• Communicate progress and issues to staff in a timely manner.
• Plan and maintain Member and household accounts.
• Maintain records and submit documentation on time.
