Qualifications

• 21 years of age or older per licensing requirements.
• Proficient in spoken and written English language.
• Comfortable providing personal cares and administering medication.
• Successful clearance of Department of Human Services background check.
• Acceptable driving record as per company policy.

Responsibilities

• Support individuals in achieving tailored outcomes.
• Assist with medical appointments and documentation.
• Provide direct support and personal cares as needed.
• Participate in community activities with individuals served.
• Manage supervision and transportation for community appointments.
