Job summary

Nevada Health Centers I is seeking a Registered Nurse Complex Case Manager to lead high‑risk patient care in the ambulatory setting. You will assess needs, plan and coordinate services with patients, families and providers to improve outcomes and cost effectiveness.

Required NV RN licensure, CPR/BLS, 3+ years in clinical practice (ER/ICU), and experience with Electronic Health Records; you will collaborate with an interdisciplinary team to implement patient‑centered care across the continuum.

Qualifications

• Graduate of an accredited nursing school; RN license required in NV.
• CPR or BLS certification required.
• Minimum three years in clinical practice (ER/ICU) with strong clinical skills.
• Familiarity with Electronic Health Record systems.
• Minimum two years experience in complex case management.

Responsibilities

• Manage complex case load for high-risk patients in the ambulatory setting.
• Coordinate services and develop care plans with interdisciplinary team.
• Monitor utilization, identify gaps in care, and ensure patient-centered outcomes.
• Liaise with physicians, hospitalists, and care facilities.
• Provide timely follow-up during patient transitions between settings.
