Qualifications

Responsibilities

• Assess patient’s physical, psychosocial, cultural, and spiritual needs through observation, interview, review of medical records, and clinical data.
• Evaluate patient’s response and report to provider for optimal outcomes.
• Contribute to care plans in collaboration with the interdisciplinary team.
• Oversee clinic staff and scheduling; manage supplies and expenses.
• Ensure patient engagement and education using motivational interviewing and teach-back methods.
