Job summary

Lakelands Nursing and Rehabilitation Center in Abbeville, SC seeks a Charge Nurse (LPN or RN) for full-time, 2nd shift. You will provide direct nursing care to residents and supervise certified nursing assistants in accordance with federal, state, and local regulations.

Responsibilities include admission/discharge and transfers, medication administration, wound care, care plan updates, and collaboration with the interdisciplinary team.

Qualifications

• Nursing degree from an accredited college or university or graduate of an approved RN program.
• Current unrestricted license as a Registered Nurse (RN) in the state of South Carolina.
• Current CPR certification.

Responsibilities

• Directs the daily activities of the certified nursing assistants in accordance with current federal, state and local regulations and guidelines and established facility policies and procedures.
• Ensures that policies and procedures are complied with by nursing personnel assigned.
• Participates in admission, discharge and transfers of residents as required.
• Observes changes in residents’ status and notifies physician and family, documenting accordingly.
• Transcribes physician orders to medical record and carries out orders as written.
• Prepares and administers medications as per physicians’ orders and monitors for adverse effects.
• Performs wound treatments as per physicians’ orders and documents results.
• Schedules follow-up appointments and transportation needs as indicated.
• Provides nursing leadership to unit staff and direct care such as colostomy changes, tube feedings, wound care, suctioning, IV administration.
• Performs rounds to ensure resident needs are met and staff duties are performed.
• Collaborates with interdisciplinary team to meet residents’ needs holistically.
• Reports incidents to supervisor and participates in investigations as needed.
• Participates in end-of-life care as required.
• Ensures adequate stock of medications and supplies; notifies personnel of needs.
• Initiates and updates care plans as required.
• Promotes teamwork, mutual respect, and effective communication.
