Job summary

City Net is seeking a Team Lead to oversee case management for individuals and families experiencing homelessness. You will help connect clients to housing, medical care, counseling, and supportive services while supporting supervisors with staff performance, scheduling, and budgets.

The role includes mentorship of Case Managers, collaboration with partners, and ensuring HMIS data accuracy. Work hours are Monday–Friday, 7am–3:30pm, with a focus on maintaining service quality.

Qualifications

Description

Position Overview

Team Leads provide case management and supportive services to individuals and families experiencing street-level homelessness, focusing on connecting them to sustainable housing and supportive resources like medical care, counseling, and mental health treatment. They coordinate care with nonprofits, government entities, law enforcement, and healthcare providers to help clients secure and maintain permanent housing. Additionally, TLs assist supervisors by monitoring staff performance, schedules, and expenses, while providing mentorship, training, and guidance to Case Managers.

Work Schedule: Monday-Friday 7am-3:30pm

Key Responsibilities
  • Support Team Supervisors in ensuring that all Case Managers (CMs) are delivering services in a professional manner and in accordance with all departmental and organizational policies and procedures.
  • Support Team Supervisors in coaching, mentoring, and training CMs as needed. Thoroughly document all coaching, mentoring, and training.
  • Support Team Supervisors in completing and coordinating special projects, as they arise.
Administrative Support
  • Support Team Supervisors with the administration and completion of case management tasks.
  • Implement team logistics and communicate with upper management on the status of projects.
  • Ensure CMs complete any and all required data entry thoroughly, accurately, and on time and follow all data entry, reporting, and submission procedures.
  • Ensure all CMs are following the current HMIS procedures and best practices.
  • At all times model excellent case management services and efficient, timely, and accurate completion of related tasks, which include but are not limited to maintaining case notes, entering data, and completing and updating IAPs.
  • Professionally represent City Net at local, County, and City collaborative meetings.
  • Cultivate a positive work environment by promoting teamwork and collaboration among staff.
  • Provide real-time guidance and training to CMs when they have questions, appear to be struggling with a client scenario and/or an internal procedure or expectation, and/or when requested.
Compliance and Goal-Meeting
  • Consistently meet all assigned productivity and outcome goals.
  • In all activities, comply with all contracts, funder requirements, and City Net procedures and policies.
  • Meet or exceed all goals and requirements established by the Chief Program Officer for this position.
  • Consistently perform Case Management responsibilities, including Client Needs Assessment, Housing Placement, Linkage to Supportive Services, Advocacy and Data Collection
Requirements
  • Case management experience with people affected by homelessness, substance use disorders, chronic health and mental health concerns, domestic- and intimate-partner violence, and/or human trafficking
  • Experience cultivating and maintaining productive, professional relationships with clients, partner agency representatives, and coworkers
  • Strong interpersonal, plan execution, and time management skills
  • Professional verbal and written communication skills
  • Ability to work as a leader and as part of a team
  • Will be required to drive a company vehicle to transport clients. For insurance purposes, must be at least 21 years of age, possess a valid driver’s license, and must pass a Motor Vehicle Report.
