HomeCentris Healthcare is seeking a Lead Caregiver in Philadelphia, PA to lead and mentor a team of caregivers. The ideal candidate holds a CNA certification or has 5+ years of caregiving experience, with CPR/First Aid and a valid driver’s license.

Regular full-time hours and a Monday–Friday plus weekend schedule offer meaningful, hands-on patient care. Responsibilities include direct personal care, coordination with care teams, and ensuring adherence to care plans.

Qualifications

• Active CNA certification or a minimum of 5 years of caregiver experience.
• Reliable transportation and a valid driver's license (must drive).
• Current CPR and First Aid certification.
• Flexibility to work throughout Central PA.
• Strong leadership skills and the ability to support clients and caregivers in the field.

Responsibilities

• Provide direct personal care including bathing, grooming, dressing, toileting, and mobility assistance.
• Assist clients with daily living activities and companionship.
• Administer or assist with medications as directed.
• Monitor clients' health conditions and report any changes to supervisors or healthcare professionals.
• Train, mentor, and support new caregivers.
• Ensure care plans are followed accurately and updated when needed.
• Maintain accurate documentation of care and services provided.
• Communicate effectively with clients, families, and care teams.
• Ensure compliance with company policies and safety standards.
