Job summary

Marin Community Clinics in California seeks a Lead Care Navigator Medical Assistant/Non-Medical Assistant to support in-clinic care navigation and patient services. You will work closely with care teams, assist with eligibility, BH referrals, and scheduling, and help connect patients to community resources.

The role involves training Care Navigators, maintaining patient confidentiality, and backfilling as needed. Bilingual Spanish/English and MS Office/Epic EHR experience are valued assets.

Qualifications

• Bilingual Spanish/English required.
• Certificate from an accredited Medical Assisting program preferred but not required.
• 1-2 years of related work experience.
• Prior experience at a community health center is a plus.
• BLS certification is required (Applicable to Certified Medical Assistant only).
• Epic Electronic Health Records experience is a plus.

Responsibilities

• Primary point-of-contact for in-clinic Care Navigators.
• Support and train Care Navigators on MCC clinical and operational processes.
• Work with Lead Care Navigators at all clinics in support of the organization-wide Care Navigation Program.
• Practice strong interpersonal effectiveness to maintain good public relations with patients, colleagues and providers.
• Maintain patient confidentiality per department policy and HIPPA.
• Attend required meetings, training and educational sessions.
• Perform administrative medication/equipment tasks: check eligibility, process BH referrals, verify demographics, help patients with paperwork, and follow-up.
• Stay current with community resources and connect patients to needed resources (transportation, etc.).
• Request Prior Authorizations and TARs from the pharmacy for timely medications.
• Support BH patients with scheduling, referrals, and crisis support when needed.
• Backfill as a Medical Assistant when required.
• Prepare treatment rooms and escort patients to exam rooms; record vitals.
• Follow OSHA Bloodborne Pathogens and Hazardous Materials guidelines.
• Perform other related work as assigned.
