Job summary

Ramey-Estep/Re-group seeks a Senior Care Navigator to lead the Care Navigation team across NKY and Georgetown offices. The role ensures timely, person-centered navigation for behavioral health, primary care, and community supports.

Responsibilities include supervising navigators, coordinating intake, and ensuring adherence to quality and regulatory standards while collaborating with clinical and community partners.

Qualifications

• Requires knowledge of community resources, health systems, and behavioral health navigation.
• Experience in behavioral health, case management, or community navigation preferred.
• Must be at least 21 years old with strong time management and organizational skills.

Responsibilities

• Provides direct supervision, training, coaching, and support for Care Navigators at the assigned office.
• Manages daily workflow, intake coverage, and client engagement responsibilities.
• Ensures coverage for in-person, telehealth, outreach, and administrative navigation needs.
• Conducts regular supervision and team meetings for accountability and growth.
• Participates in hiring, onboarding, and evaluation of staff.
