Job summary

Ramey-Estep Homes, Inc. is seeking a Senior Care Navigator to lead the Care Navigation team across assigned offices. You will oversee supervision, workflow, coverage for in-person and telehealth, and ensure services meet quality standards and client needs.

You will collaborate with clinical leadership, case management, and partners to reduce barriers and improve engagement and stability for clients, while maintaining regulatory compliance.

Qualifications

• Knowledge of community resources, health systems and behavioral health navigation required.
• Experience in behavioral health, case management, or community navigation preferred.
• Must be at least 21 years old and have a valid driver's license.

Responsibilities

• Direct supervision, training, coaching for Care Navigators and staff.
• Assigns workflow, intake coverage, referrals, and client engagement.
• Ensure in-person/telehealth navigation coverage and timeliness.
• Conduct regular supervision and team meetings for accountability.
• Assist hiring, onboarding, and evaluations for care navigation staff.
• Monitor productivity, documentation, and follow-up outcomes.
• Oversee screening, intake processes, and client education on services.
• Coordinate access to behavioral health and primary care services.
